Facility:

This established healthcare organization is known for their compassionate approach to palliative and hospice care. Their outpatient palliative care services extend across a 9-county area in Northwest Ohio, providing critical support to patients facing serious illnesses through a collaborative, interdisciplinary care model.

Opportunity: Board Certified - Nurse Practitioner

This is an excellent opportunity for an experienced Nurse Practitioner to join a supportive team providing meaningful outpatient hospice and palliative care services across Findlay and surrounding counties in a full-time permanent position.

Job Details:

• Provide outpatient palliative care services including home visits, extended care facility consultations, telehealth, and clinic-based care
• Manage 6-8 patients per day across a 9-county area in Northwest Ohio
• Conduct goals-of-care discussions with patients and families facing serious or terminal illnesses
• Assess, plan, implement, and evaluate individualized care needs following palliative care principles
• Collaborate with an interdisciplinary team of NPs, physicians, and administrative staff
• Participate in determining when patients would benefit from continued palliative care or hospice services
• Work closely with the hospitalist team responsible for patient admissions
• Maintain thorough documentation and excellent communication with patients, families, and care teams
• Ohio license required (or willingness to obtain)
• Experience as an advanced provider in acute care or palliative/hospice care required
• Travel required across service area (reliable personal transportation needed)
• No weekends or call required

Compensation:

Competitive hourly rates with weekly pay and malpractice included. The comprehensive package also features quality bonus opportunities, paid mileage for travel, CME allowance, and a full benefits package.

Estimated Annual Salary: $120000 - $140000
